Systemic Inflammation Response (SIRS)
Exadurated response to a stressor or trauma, infection, acute inflammation
Non-specific infection
SIRS Criteria
HR > 90 BPM RR > 20 BPM Temp > (or equal to) 38 OR < 36 WBC > 12.0 or < 4.0 x 10 g/L Altered mental status (<15 GCS)
Key interventions of SIRS/sepsis
- Labs and diagnostics
- IV fluids
- Antibiotics
- monitoring criteria
SEPSIS
If suspected or known pathogen is identified
Suspected or confirmed source of infection + SIRS criteria
Risk factors of SIRS and suspected source of infection
Increased or decreased age
immunosuppressant drugs
comorbidity
Invasive meds or treatments
Severe Sepsis
patient is showing signs of hypoperfusion to one organ system
causes multiorgan failure
(sepsis + hypoperfusion to 1 organ)
Septic Shock
severe sepsis with hypotension that does not resolve despite IV fluid resuscitation
Labs and Diagnostics
Blood culture (local or systemic infection?) serum lactate (chemical naturally produced by the body to fuel cells during times of stress)
IV fluids
IV bolus, secondary IV access
Make sure to repeat VS and chest ausc. prior and following each fluid bolus
Antibiotics
Wait 30 minutes before administering IV antibiotics if the blood culture isnt ready yet
