Compensated Shock
-Agitation, anxiety, restlessness
-Impending doom
-Normal to slightly increased HR
-Weak, rapid (thready) pulse
-Clammy (cool, moist) skin
-Pallor w/ cyanotic lips
-Delayed cap refill (infants & peds)
-BP is maintained
-Tachypnea
-SOB
-Nausea/vomiting
-Thirst & oliguria
Decompensated
-Anxiety or ALOC
-Impending cardiac arrest
-Dilated pupils
-Hypotension
-Labored or irregular breathing
-Thready or absent peripheral pulses
-Ashen, mottled, pale, cool, clammy or cyanotic skin
Modified qSOFA (or quickSOFA) for Sepsis
Respiratory rate greater than 22 breaths/min
Systolic BP less than 90 mm Hg
