- Tenderness over transverse process f vertebra
- Radiation of pain to posterolateral arm
- Numb sensation 2nd and 3rd digits
- Triceps reflex reduced or absent
- Motor weakness and atrophy of triceps muscle

- Radiation of pain to anterior thigh and leg

- Motor weakness and atrophy of quadriceps muscle

- Motor weakness and atrophy of plantar flexors

- Pain in gluteal region between ischial tuberosity and greater trochanter
- Tenderness over transverse process of vertebra
- Radiation of pain to lateral thigh and leg
- Numbness sensation of lateral leg, three middle toes
- Reflex’s are intact
- Motor weakness and atrophy of dorsal flexors of ankle and toes

A
- Pain in medial gluteal fold
- Tenderness over upper lateral sacrum
- Radiation of pain to posteriomedial thigh, leg and heel
- Numbness of medial gluteal fold
- Reflex of plantar flexion reduced or absent
- No Motor weakness or atrophy recognized
