Schizophrenia Spectrum and Other Psychotic Disorders
- DURATION make it different
- five domains are delusions, hallucinations, disorganized thinking (usually in speech), grossly disorganized motor behaviors and negative symptoms
Delusions
- fixed beliefs that are not amenable(適合) to change in light of conflicting evidence
- these are fixed and set in stone, rigid cannot be broken
- persecutory, grandiose, erotomanic, nihilistic,somatic
Hallucinations
-perception-like experiences that occur without an external stimulus
Disorganized Thinking(speech)
- the individual may switch from one topic to another, answers to questions may be obliquely(不直接的) related or completely unrelated
- they are speaking fluently but the context makes no sense
Grossly Disorganized or Abnormal Motor Behavior
-may manifest in a variety of ways, ranging from childlike “silliness” to unpredictable agitation
Negative Symtoms
-two negative symptoms are particularly prominent in schizophrenia: diminished emotional expression and avolition(decrease in motivated self-initiated purposeful activities)
Delusional Disorder
- the presence with a duration of one month or longer
- even they know the consequences, they still wanna do it
Brief Psychotic Disorder
- presence of at least one of the following: delusions, hallucinations and disorganized speech
- THESE THREE ARE THE HALLMARK OF ALL THESE DISORDERS
- must have one of these symptoms and then it must disappear and then they go back to normal functioning
- lasting at least one day but less than 1 month
Schizophreniform Disorder
- two symptoms in total, one from of the following delusions, hallucinations, disorganized thinking and the other can be other options
- lasting at least 1 month but less than 6 months
Schizophrenia
- two symptoms in total, one from of the following delusions, hallucinations, disorganized thinking and the other can be other options
- lasting at least 6 months
- sometimes able to go back to premorbid functioning but since their big psychotic break they never get back to normal really
- cycles through prodromal active and residual phases
Schizoaffective Disorder
- meet criteria for schizophrenia (A)
- lasting at least 6 months
- have to have a major mood episode co occurring with those symptoms
- YOU HAVE PSYCHOSIS AND SOMETIMES MOOD BUT YOU CAN HAVE PSYCHOSIS BY ITSELF
Differences between Psychotic Disorder and Mood Disorder
- MOOD DISORDERS WITH PSYCHOTIC FEATURES YOU HAVE MOOD BY ITSELF BUT NEVER PSYCHOTIC FEATURES BY THEMSELVES
- SCHIZOPHRENIA DOES NOT HAVE THE MOOD DISORDER PRESENT
Catatonia
stupor(麻木), catalepsy(全身僵硬), waxy flexibility, mutism(緘默), negativism, posturing(故作姿態), mannerism(怪癖), stereotypy, agitation, grimacing(鬼臉), echolalia(模仿言語), echopraxia(模仿行為)
