What is a PAE?
Proceed aspect exceeded - Where a Driver enters a section without the correct authority to do so.
What is the drivers’ responsibility if having a suspected PAE?
- immediately bring train to stand and secure
- Assess the situation in relation to risk to other trains
- If risk to other trains make a REC call
- Do not move the train
- Contact controlling signaller or train controller
- Provide information
- Follow instructions
What are the causes of a PAE?
- distraction
- Fatigue
- Impaired by drugs or alcohol
- outside of work distractions
What are 4 kinds of Safety Critical incidents?
- platform stopping errors
- Wrong side door openings
- Derailments
- Train rolling backwards/forwards with saloon doors released
What is Threat and Error Management?
Utilising skills to avoid turning threats into errors.
What are some causes of safety critical incidents?
- being distracted
- Lapse of concentration
- Missing a medium speed warning
- Not following correct procedure
- Misjudgment/not allowing for margin of error
What defines a multi PAE signal?
Any signal that has been passed three times and a five year period.
Threat?
Can be described as anything that may have the potential to cause an error
Error?
Can be described as a mistake that may lead to an undesired state.
Undesired state?
And undesired state is the safety critical incident.
