Describe the purpose of an Accident/Near Miss Investigation? Where can you find the form to be used? And who is to conduct?
- The accident near miss form is used to dig deep into what happened to prevent an injury in the future and eliminate the root cause of the incident.
- Identify the situations and circumstances surrounding an accident or near miss.
- The form can be found in the safety center and front office.
- The supervisor or manager of the partner involved in the accident or injury should complete the investigation. Follow up should be done with the affected partner and anyone who works in the same area.

- To dive into the root cause of an incident and develop solutions.

What is the function of the Safety and Improvements Committee? Who should be members?
- The Safety and Improvements Committee encourages awareness to get partners actively involved in the safety process, helps train partners to follow safety practices and improve safety process at the location.
- The committee should be comprised of partners from Production, Service, Office, and Sales
What is the most frequent cause of injury at a Cintas location?
• Strain injuries from lifting, pulling, twisting
Two components of a location’s safety program are compliance and safety – explain each and how they can affect your operation.
- Compliance deals with meeting the requirements of state and federal laws.
- You have OSHA compliance- deals with issues of safety training, injury reporting, safety equipment and PPE, etc.
- Environmental compliance-deals with wastewater pretreatment, sludge disposal, air quality. Failing to maintain compliance in either area can result in fines and can result in our facility being shut down.
- Safety compliance- Deals with partner training and partner understanding of our safety program. CSOP’s, fire, electrical, LOTO

What is the difference between an OSHA recordable and first aid incident?
- An incident is OSHA recordable if the partner receives medical treatment beyond first aid, is given prescription medication, or the injury results in lost time from work.
- If the partner merely receives a non-prescription medication and no medical procedure is performed, the incident is a first aid incident and is not recordable.
If a partner sustains a workplace injury and wants to go to the doctor, who should take the partner to a medical facility?
- HR manager/ PM/ Supervisor/leadership

What are the benefits of Stretch & Flex and how often should it be conducted?
- Helps warm up the muscles to reduce injury from muscle tightening, improve health and wellbeing, promote safety.
- Decreases the chances of strains or pulled muscles
- Should be done at start of shift, after each break and throughout the day when needed
What is the number one cause of property losses at our facilities each year?
• Fire & smoke damage
What are the two primary areas at Cintas where fires occur?
- Bulk storage- Dirty and clean (shop towels and mops)

What is spontaneous combustion? What materials in our laundries are subject to spontaneous combustion?
- Occurs when product, dirty or clean, increases in temperature though oxidization without drawing heat from its surroundings and catches fire.
- Dirty linen
- Clean shop towels- if not dried properly or washed on the correct formula
- Clean mops when washed on the wrong formula

Name several key components of a good fire prevention program:
- Housekeeping
- Fire prevention plan in place
- Emergency action plan in place
- Effective daily/weekly/monthly/annually fire inspections

- Partner training on proper fire extinguisher use
- Proper storage of soil goods containing flammable product
- Proper maintenance of laundry equipment

- 0-Oblivious- Location has basic understanding of what safety is.
- 1- Defiant- Legal compliance (Looking at safety as a check list)
- 2- Resistive- Cost of injuries ($ amount associated with injuries and property damage)
- 3- Cooperative- Safety is a top priority
- 4- Dependable- Safety is a value
- 5- Committed- Safety is instinctive (Culture of safety at location)
