How many patient suffer right sided MI and what are the concerns involved with it?
approximately 40% of ST elevation MI will have right sided involvement.
Can be a concerns when administering nitrates due to hypotension.
How do I recognise Right sided MI ?
- ST elevation in LIII is greater than in LII
- STD in V2 accompanied with STE or isometric STE in V1
- Once ECG leads changed > 0.5mm STE IN V3R and V4R
What leads can right sided MI be present in?
Refer to recognition
However when in doubt move V4 to mirror opposite side and V3 can be moved too.
- Remember to change the name from V4/3 TO V4R/V3R
Signs and symptoms of Right sided MI involvement?
Hypotension
chest pain
Jugular distension
shock
can be asymptomatic
women may present different
concerns of right sided MI?
Refer to guidelines as administration of nitrates and certian analgesia can cause significant preload complications causing hypotension and possible circulatory collapse.
