How many patient suffer right sided MI and what are the concerns involved with it?

A

approximately 40% of ST elevation MI will have right sided involvement.

Can be a concerns when administering nitrates due to hypotension.

How do I recognise Right sided MI ?

A
  • ST elevation in LIII is greater than in LII
  • STD in V2 accompanied with STE or isometric STE in V1
  • Once ECG leads changed > 0.5mm STE IN V3R and V4R

What leads can right sided MI be present in?

A

Refer to recognition

However when in doubt move V4 to mirror opposite side and V3 can be moved too.

  • Remember to change the name from V4/3 TO V4R/V3R

A

Hypotension
chest pain
Jugular distension
shock

can be asymptomatic
women may present different

A

Refer to guidelines as administration of nitrates and certian analgesia can cause significant preload complications causing hypotension and possible circulatory collapse.
