  • due to diminished capacity of the proximal tubule to reabsorb bicarbonate
  • appearance of HBO3 in the urine
  • low plasma HCO3 and acidic urine
  • their is a significant amount of fixed acid lost in the urine
  • FANCONI SYNDROME a defect in the proximal tubular transport process and carbonic anhydrase inhibitors

  • due to inability of the distal nephron to secrete and excrete fixed acid
  • inability to form an acid urine
  • urine pH > 5.5 - 6
  • impairment transport of H+ and HCO3
  • metabolic acidosis
