Outlline a responsible pharmacist notice
- Must be displayed prominently
- Should be typed so that it is easy to read
Outline the responsible pharmacist record book
- Legally required under The Medicines (Pharmacies) (Responsible
Pharmacist) Regulations 2008 - the Responsible Pharmacist’s name;
- any number associated with the responsible pharmacist’s registration
in the Register of Pharmacists maintained under Pharmacy Order
2010 - the date and time at which the responsible pharmacist became the
responsible pharmacist; - the date and time at which the responsible pharmacist ceased to be
the responsible pharmacist;
Outline absence of any responsible pharmacist
In relation to any absence of the responsible pharmacist from the
premises on a day on which they were the responsible pharmacist;
- the date of the absence;
- the time at which the absence commenced; and
- the time at which the responsible pharmacist returned to the
premises.
Outline regrigerator records - fridge temperatures
- A daily log of the temperature of the fridge
- Temperature should be kept between 2 and 8 degrees Celcius
- Record is kept to meet contractual requirements regarding the safety
of medicines - Record will also allow pharmacists to determine whether a medicine
in the fridge is safe to supply or not - Medicines stored in Fridges can become less effective if they get too
hot or are frozen

Legally required under The Human Medicines Regulations 2012
* the date on which the prescription only medicine was sold or supplied;
* the name, quantity and, except where it is apparent from the name, the
pharmaceutical form and strength of the prescription only medicine sold or
supplied;
* the name and address of the person giving the prescription;

Briefly outline controlled drugs register
- Legally required under The Misuse of Drugs Regulations 2001 (as amended)
- An entry must be made for purchases and supply of schedule 2 controlled drugs
Outline 1 requirements on a controlled drugs register and requirements for PURCHASES
HEADINGS
* Every page of the register must specify the name of the drug and the strength
and form (i.e. a different page for every variation in strength, name/brand name
and form of a drug
PURCHASES
* Date on which supply received
* Name and address of person or firm from whom obtained (i.e. who you bought
the drugs from)
* The amount obtained
* Form in which obtained (pharmaceutical form)
Outline requirements for SUPPLIES on a controlled drugs register
SUPPLIES
* Date on which transaction effected (day of supply)
* Name and Address of person or firm supplied
* Particulars as to licence or authority of person or firm supplied to be in
possession (who prescribed it)
* person collecting a Schedule 2 CD (patient, representative, or healthcare
professional) and if a healthcare professional, their name and address
* whether proof of identity was requested (yes or no)
* whether proof of identity of person collecting was provided (yes or no)

Outline patient medication records
- The Terms of service of NHS pharmacists within The National Health
Service Regulations
2013
Requires Pharmacists to: - Keep and maintain records of drugs and appliances provided, in order to
facilitate the continued care of the patient; - in appropriate cases, of advice given and any interventions or referrals
made - This can be met by using the PMR system in the pharmacy to record any
supplies made as part of the dispensing process
Outline records relating to service provision
Records of interventions made by the pharmacist when providing the
following NHS services should be made
* to prove pharmacist engagement and compliance with the service
* AND make sure information that is held in the interests of the patient
is maintained
* Services where it is sensible to make records include
* Signposting
* Support for self care
* Promotion of healthy lifestyles
