pathophysiology of septic arthritis?
infection starts in bone> spreads to periosteum > engulf joint capsule
aetiology of septic arthritis
trauma, inoculation, metaphyseal spread, haemtogenous (usually from osteomyelitis)
s/s of septic arthritis?
monoarthropathy, hot, painful, swollen, red joint, reduced ROM/ struggling to walk, fever
exam findings for septic arthritis?
erythema, effusion, warm, tender
gold standard ix for septic arthritis?
aspirate synovial fluid
what is the criteria used for septic arthritis?
Kocher’s Criteria: >38.5º, refusal to wt bear, CRP/ESR (40mm), WBC >12,000
tx for septic arthritis?
surgery: drain pus, washout
6 week abx course
morbidity of septic arthritis?
may result in irreversible damage to hyaline cartilage resulting in early arthritis
what is periprosthetic joint infection (PJI)
an infection of a prosthesis that may occur after an arthroplasty
pathophysiology of PJI?
infection occurs between cement and bone and causes loosening of mechanics and pain
