what is soft tissue rheumatism?
general term to describe pain that is caused by inflammation/ damage to ligaments, tendons, muscles or nerves near a joint
s/s of soft tissue rheumatism?
localised pain at specific site
what’s the difference between soft tissue rheumatism and fibromyalgia in terms of presentation?
STR: specific localised pain
fibromyalgia: generalised soft tissue pain
common sites for soft tissue rheumatism?
neck, shoulder, elbow, wrist, pelvis, foot
ix for soft tissue rheumatism?
x-ray, mri
tx for soft tissue rheumatism
conservative- analgesia, rest, ice compress, physio, steroid injections
what is carpal tunnel syndrome?
relative ischaemia of medial nerve
T/F: carpal tunnel is common in women >60
F: women >30
aetiology of carpal tunnel?
pregnancy, hypothyroidism, diabetes, obesity, RA, acromegaly, fluid overload
pathophysiology of carpal tunnel?
compression of median nerve at wrist due to hypertrophy or oedema of the flexor synovium
which muscles does the median nerve supply?
LOAF Lumbricalis IF & MF Opponens Abductor Pollicis Brevis Flexor Pollicis Brevis
where would you assess medial nerve function in hand?
palmar aspect hand, thumb, index, middle and radial half of ring finger
s/s of carpal tunnel?
pins & needles- esp in morning, pain, clumsiness, fingers feel swollen- relief of symptoms by shaking hand
what are late signs of carpal tunnel?
weakness and numbness
ix for carpal tunnel?
exam: thenar atrophy, altered sensation, weakness
ix: Kamath & Stothard questionnaire, EMG
what are the 3 specialised tests for carpal tunnel?
Tinel’s test, Phalen’s test,Durkin’s Test
mx for carpal tunnel?
mild- splint, physio, steroid injections
severe- carpal tunnel decompression surgery (divides transverse carpal ligament)
what is cubital tunnel syndrome?
neuropathy and relative ischaemia of ulnar nerve usually as a result of compression of elbow region
do males or females tend to get cubital tunnel syndrome?
males
aetiology of cubital TS?
trauma to medial epicondyle, direct pressure, arthritis, medial epicondylitis
which muscles does ulnar nerve supply sensation to?
all other nerves except LOAF
s/s of cubital TS?
pins and needles, pain in ring and little finger, clumsiness
ix for cubital TS?
exam: hypothenar and interosseous atrophy, clawing of ring and small finger, altered sensation, weakness- grasp/pinch
ix: nerve conduction studies
what is the signs and specialist tests for cubital TS?
Wartenberg’s sign (abduction of small finger)
Tinel’s, Phalen’s, Froment’s sign
mx for cubital TS?
mild- elbow splint, physio, NSAIDs
severe- ulnar nerve decompression, medial epicondylectomy
