What is the definition of a fever?
Elevation of body temperature above normal (37c)
Variation of up to 0.8c daily, low in morning, high in early evening
What is a pyrogen?
Substances which cause fever
What are the different types of PUO?
Classical PUO
Nosocomial PUO
Neutropenic PUO
HIV associated PUO
What is nosocomial PUO?
Develops in hospital, undiagnosed after 3 days
What is neutropenic PUO?
Undiagnosed fever in patient with neutrophils <500/mm3
What is HIV associated PUO?
Fever in a patient with HIV infection, present and undiagnosed for more than 3 days inpatient or 4 weeks outpatient
What are the causes of classical PUO?
Infection
Malignancy
Inflammatory
What are the common causes of HIV associated PUO?
Mycobacterium tuberculosis
Mycobacterium avium
Unknown
Cytomegalovirus
What initial investigations should be done for PUO?
CXR Urinalysis and urine microscopy FBC and differential WCC CRP and ESR Blood cultures taken at different times Urea, creatinine, LFTs, electrolytes
What investigations should be done for recent tropical travel?
Blood for malarial parasites, dengue, HIV
Bone marrow for leishmaniasis
What investigations should be done for a new murmur?
Echocardiography
What investigations should be done for headaches?
Temporal artery biopsy
What investigations should be done for microhaematuria?
Auto-antibodies and renal biopsy
USS renal
What investigations should be done for TB contact?
Sputum smear
Bone marrow
Mantoux test
What is a fabricated fever?
Fever is real but self-induced
