What is a functional disease
Function of the affected organ is histogically fine but is functioning abnormally
What is the relationship between how long after you eat, you vomit and the condition being organic or functional
If you vomit right after you eat, it is probably functional but if you voimit few hours after you eat it is probably an organic problem
What is refeeding syndrome
Body becomes used to syate of starvation so when the carbohydrates are given at too quick a pace for the adapted body, there is a rapid rise in insulin which can cause hypophosphataemia
What is the criteria to determine high risk of refeeding syndrome
if patient has one of these:
BMI less than 16 kg/m2
unintentional weight loss greater than 15% within the last 3–6 months
little or no nutritional intake for more than 10days
low levels of potassium, phosphate or magnesium prior to feeding.
If patient has 2 of these:
BMI less than 18.5 kg/m2
unintentional weight loss greater than 10% within the last 3–6 months
little or no nutritional intake for more than 5 days
a history of alcohol abuse or drugs including insulin, chemotherapy, antacids or diuretics.
What is the treatment of someone who is at risk of refeeding syndrome
Start slowly:
- IV fluids
- Thiamine 30 mins before feeding
- feed slowly and gradually increase over a week
- Replace phospate, K, Mg and thiamine
Describe binge eating disorder
Binges and purges(vomits or laxatives…) but fails to compensate so weight gain
Describe bulimia nervosa
Restriction and then binging episodes with purging but compensated well so normal weight
Describe anorexia nervosa
Restriction which causes severe weight loss
Fear of weight gain due to body dysmorphia
Over exercising
Can have amenorrhoea in women
