Bacterial Infection

What is the most common type of organism for prostatitis?
A
- GN bacteria
- MC: E. coli
- other buds: proteus, psudomonas aeruginosa

Mechanism of infection?
A
- ascending through urethra
- can involve bladder and epididymis, orchitis as well

RF for prostatitis?
A
- Repeated UTI
- recent instrumentation
- bladder catheterization (chronic indwelling or clean intermittent)
- recent prostate bx

General presentation of the disease?
A
- Pt sick: acutely ill with fevers, chills, fatigue, malaise, and myalgias
- Lots of urinary symptoms

How does the prostate feel?
A
warm, firm, edematous, exquisitely tender

1st line of Abx if not admitted
A
- Bactrim (TMP-SMX)(if P. aeruginosa not suspected)
- Cipro
- Levo

1st line if admitted (systemic symptoms)
A
- IV cipro or levo

Should you ever use nitrofurantoin for prostatitis?
A
NO - poor penetration

What are the Abx for P. aeruginosa?
A
- Cipro, Levo
- Pen: Zosyn and Timentin
- Cephalosporin: Ceftazidime, Cefepime, Cefoperazone
- Monobactams: Aztreonam
- Carbapemen: imi-, mero-, dori-
- Aminoglycoside: Genta, Tobra, Ami
- Polymixins: Colistin
