Primary Health Care & Population Health Promotion in Canada Flashcards
Primary health care
- > it is essential care that focuses on preventing illness and promoting health
- > refers to a health system which is composed of a core set of functional and structural elements that guarantee universal coverage that includes accessible healthcare for all individuals
essential health care services
- > promotive care
- > preventative care
- > curative care
- > rehabilitative care
- > support care
Foundation of Canada’s healthcare system
- > universally accessible
- > it is essential care that focuses on preventing illness and promoting health
- > composed of core functional and structural elements that guarantees universal coverage that includes accessible healthcare for all individuals
Health care delivery: primary health care
- > it is a philosophy and a model for improving health that supports essential health care services with a strong emphasis on the principles of health promotion and disease prevention
- > In PHC, the importance of the determinants of health and strategies to advance individual and population health is recognized.
Primary Care
*NOT THE SAME AS PRIMARY HEALTH CARE, THOUGH USED INTERCHANGEABLY*
- > direct provision of first contact services by providers such as family physicians and nurse practitioners
Goals associated with Primary Health Care
- > address equity issues
- > provide relevant/accessible programs/services
- > situate health intersectorally
- > build collaborative models for program/policy dialogues
- > embed patient, stakeholder and partner participation (why would u make a plan without the patient)
list all principles of primary health care
- Access
- Teams
- Information
- Healthy Living
explain the access principle of PHC
- > reducing exclusion and socialdisparities in health by ensuring that all individuals have access to appropriate services when and where they are needed
Explain the “teams” principle of PHC
- > integrating health into all sectors will improve access and coordination of care

Explain the “information” principle of PHC
- > promotes ease of access to health information through the use of innovative tools and technology such as EMR’s and Maple
EMR’s vs Maple
EMR
- > elecctronic medical records
- > many different types/programs are used by different physicians, which can sometimes cause problems
Maple
- > virtual health care service; virtual doctor
Explain the “healthy living” principle of PHC
- > healthy living embraces a focus toward health promotion and prevention, chronic illness management, and self care while recognizing the influence of other determinants of health
What is helath promotion
the process of enabling individuals to increase control over and improve their health
- > increasing levels of wellness and self-actualization
How do we promote health/acheive health promotion
- Build healthy public policy
- Create supportive environments
- Strengthen community action
- Develop personal skills
- Reorient health services
What is disease prevention and what are the three levels of DP
- > action to avoid illness, disease and its effects
1. Primary Prevention
2. Secondary Prevention
3. Tertiary prevention
Primary prevention
stop accidents before they occur (i.e. vaccines, no running on pool deck)
Secondary Prevention
early detection of disease so hat prompt treatment can be initiated to halt disease and limit disability (i.e. cancer screenings)
Tertiary Prevention
initiated in the convalescence stage of disease, directed towards minimizing residual disability and helping individuals live productively with limitations
- > i.e. maintenance, rehab/health restoration
Population Health Approach
an approach to health that aims to improve the health of the entire population and to reduce health inequities among population groups

convalescence
time spent recovering from an illness or medical treatment; recuperation.
“a period of convalescence”
