Pregnancy Clinical Sessions Flashcards
What is the largest issue for obstetric morbidity?
Obesity
What conditions are first time mums most at risk for?
Pre-eclampsia
What is the purpose of screening?
Identify the “at risk” group in whom it is hoped to improve health outcomes (do more benefit than harm)
What is the cellular basis of pre-eclampsia?
Poor trophoblast invasion + endothelial cell dysfunction
What is the cure for pre-eclampsia?
Delivery
Which features are measured in the third trimester to assess the foetal growth?
Foetal growth
- Measurements and graphs
- Biparietal diameter, Head circumference, Abdominal Circimference, Femur length
- Est. foteal weight
Which features are measured in the third trimester to assess the foetal wellbeing?
Foetal wellbeing
- Amniotic fluid volume
- CTG
- Doppler - umbilical and cerebral arteries
- (Venous doppler)
A healthy placenta has ____ resistance to flow
Low
An unhealthy placenta will show ____ end diastolic velocity on doppler analysis
Absent-reversed end diastolic velocity
What are the causes of SGA?
- Placental insufficiency (60%)
- Constitutionally small (30-40%)
- Intrinsic foetal (5-10%)
Why is MCA doppler important?
Can show us if the foetus is compensating (increased blood flow to brain)
Normal foetal blood flow should show high/low diastolic velocities because this shows a low/high resistance circuit
High
Low
