24 hr nursing and medicine and all diagnostic services, at least 3 hrs therapy per day (5-6 days/week)
Inpatient rehab (Care Setting)
24 hr nursing, daily MD presence, some diagnostic services
Skilled nursing facility (Care Setting)
24 hr nursing, MD on call, few diagnostic services, daily care provided by certified nursing assistants (CNA)
SNF, transitional care unit, subacute rehab, nursing home
Reduced ratio of nursing care, but nurse cares for more patients
Home care (Care Setting)
intermittent nursing, no diagnostic services
Outpatient (Care Setting)
outpatient services, primarily therapy services
