EX: diabetes, blood pressure, stroke
Impairment (ICF)
problem in body structure of function
EX: endurance, strength
Activity limitation (ICF)
difficulties executing tasks or actions
EX: gait, balance, jumping, sit to stand
Participation restrictions (ICF)
problems in involvement in life and social roles, activities of daily living
EX: unable to work, unable to exercise, etc.
Environmental factors (ICF)
parts of a person’s life/surroundings that affect them
EX: having stairs, what kind of surface they walk on, how they travel
Personal factors (ICF)
conditions of a person that affect their activities and healing
EX: willingness to move, fear, pain, mental state
Qualifiers (ICF)
record the presence and severity of a problem in functioning at the body, person and societal levels
Performance qualifier (ICF)
describes what an individual does in his or her current environment
Capacity qualifier (ICF)
describes an individual’s ability to execute a task or an action; indicates the highest probable level of functioning in a given domain at a given moment
If capacity < performance (ICF)
The person’s environment helps to facilitate performance
If capacity > performance (ICF)
The person’s environment is a barrier to performance
Examination (PMM)
Medical history (Hx), current condition, chief complaint (c/c), review of body systems (ROS), tests and measures, social and occupation Hx, environmental factors, personal factors, medications, tests/measures
Evaluation (PMM)
analysis of findings, generate a problem list, prioritize, clinical reasoning
Diagnosis (PMM)
PT diagnosis (Dx) is the impact of the condition on the function, which is different than the medical diagnosis; what is keeping them from moving normally
Prognosis (PMM)
predictions, improvement, # of visits, length of stay (LOS), plan of care (POC), at what point will patient reach optimal level of function/participation
