collect essential information to help resolve the symptoms/complaints, open ended questions, subjective and objective information, 4 key questions, SCHOLAR-MAC
What are the FOUR KEY QUESTIONS?
- who is this product for?
- what medical conditions does the patient have, if any?
- what medications/herbals/supplements is the patient taking, if any?
- what allergies does the patient have, if any?
Describe: ASSESS step of the PPCP in Self-Care
analyze and establish goals with four steps:
1. medication assessment
2. patient history and risk assessment
3. preventive care assessment
4. exclusions for self-treatment
Describe: PLAN step of the PPCP in Self-Care
individualized, patient-centered, collaboration with health care professional plan, 3 pathways: recommend OTC product, refer patient to healthcare professional, recommend self care management until provider is consulted
Describe: IMPLEMENT step of the PPCP in Self-Care
implement a care plan with other health care professional and the patient/caregiver, discuss care continuity
Describe: FOLLOW-UP step of the PPCP in Self-Care
often difficult in a self-care setting, but if follow up is possible the pharmacist should start the PPCP model again by collecting information
