What is a pneumothorax?
Air in pleural space
What are the symptoms of a pneumothorax?
Dyspnoea
+/- pleuritic chest pain
What are the signs of a pneumothorax on examination?
Affected side
- reduced expansion
- reduced breath sounds
- hyper resonant
Where can the air come from?
Lung
- primary
- secondary
- iatrogenic
Through chest wall
- trauma
- iatrogenic
What is a primary pneumothorax?
Spontaneous accumulation of air in the pleural space
Who commonly gets a primary pneumothorax?
Tall thin young men
How does a primary pneumothorax commonly occur?
Small sub-pleural bulla that bursts
What is a secondary pneumothorax?
Accumulation of air in the pleural space due to underlying lung disease
What can cause a secondary pneumothorax?
COPD
Asthma
Lung cancer
Fractured rib
What is a tension pneumothorax?
Pneumothorax where air can enter the pleural cavity but can not leave
What are the symptoms of a tension pneumothorax?
Severe dyspnoea
Pleuritic chest pain
Fatigue
What are the signs of a tension pneumothorax?
Tachycardia
Hypotension
Raised JVP
Deviated trachea
How is a tension pneumothorax managed?
Insert a cannula into 2nd IC in mid clavicular line
How is a pneumothorax managed?
Aspiration
Chest drain
Where is the chest drain inserted?
Safe triangle = mid axillary line of 5th ICS
Borders
- anterior edge of lat dorsi
- lateral edge of pec major
- line superior to nipple
- apex of axilla
