Boundaries of Perineum
Anterior:
* Symphysis pubis
* Inferior pubic ligament
* Ischial tuberosities
Posterior: Coccyx
Lateral: Ischeopubic rami
Posterolateral: Sarcotuberous ligament
Perineum is divided into:
- Anal triangle posterioly
- Urogenital triangle anteriorly
Contents of anal triangle
- Anal canal
- 2 ischeorectal fossae, 1 on each side of the anal canal
Contents of urogenital triangle
- External genitalia
- Superficial & deep perineal pouches
- perineal membrane
shape of ischo-rectal fossa
wedge shaped fossa (apex, base & 4 walls)
Apex of ischo-rectal fossa is formed by
levator ani
Base of ischo-rectal fossa is formed by
Fascia and skin
Medial wall of ischo-rectal fossa is formed by
Upper 2/3: (sloping edge) Levator ani
Lower 1/3: (vertical edge) external anal sphincter
Lateral wall of ischo-rectal fossa is formed by
- Obturator internus & its fascia
- its fascia forms pudendal canal enclosing pudendal nerves & internal pudendal vessels
Anterior wall of ischo-rectal fossa is formed by
- all Perineal muscles (superficial, deep & transverse)
- Perineal membrane
Posterior wall of ischo-rectal fossa
Sacrotuberous ligament overlapped by gluteus maximus

- Inferior rectal vessels
- Inferior rectal Nerve
- Perineal Artery

Course of inferior rectal vessels in ischorectal fossa
- Arise from internal pudendal A
- Cross fossa from Lateral to medial to reach anal canal
Course of inferior rectal N in ischorectal fossa
same as Inferior rectal vessels, cross fossa from laterial to medial
Course of perineal Artery
- Arises from Internal Pudendal A anterior end
- peirces medial wall of pudendal canal
- enters ischiorectal fossa
- pierces perineal membrane to enter superfical perineal pouch
