What is a Peptic Ulcer?
Damaged lesion of the Gastric/duodenal mucosa where the protective layer has been destroyed
Main Causes: (3)
- Excess acid
- H.Pylori
- Lack of mucus
Type 1
Polypoid
Type 2
Fungating
Type 3
Ulcerated
Type 4
Infiltrative
Who does it affect most?
The elderly
Which is more common, duodenal or Gastric?
Duodenal (4 times more common)
Main Risk factors: (4)
- H.Pylori
- Smoking
- NSAIDs
- Reflux
Symptoms: (4)
Pain, Weightloss, Pain on eating, Haematemesis
What is the main difference from reflux?
Pain on hunger and eating
Signs: (6)
Epigastric pain, Anaemia, Malaena, Tachycardia, Tachypnoea, Hypotension (if actively bleeding)
Main Differentials: (2)
Non-Ulcer Dyspepsia or Malignancy
Investigations (3)
H.pylori stool test, Endoscopy, Ulcer Biopsy
Drug Treatment
Triple Therapy with Amoxicillin, Clarithromycin/Metronidazole and PPI
