What causes obstruction of the Small bowel? (2)
Adhesions and Hernias
What causes obstruction of the large bowel? (4)
Colon cancer, constipation, diverticular disease, volvulus
What else can cause either small or large?
Ileus/motility disorder of the bowel
What is a simple obstruction?
One obstructing point with no vascular compromise
What is a closed loop obstruction?
Obstruction at 2 points forming a loop of distended bowel
What are the risks with closed loop obstruction?
Risk of perforation
What is a Strangulated obstruction?
Blood supply is compromised
Symptoms (5)
Vomiting (faeculent vomiting), nausea, anorexia, colic, constipation
Signs (2)
Abdominal distension, tinkling bowel sounds
Investigations (4)
Abdominal X-ray, CT scan, PR exam, Bloods (FBC=WCC)
First line treatment
Surgery to remove obstruction
Other Treatment (5)
Analgesia, NG tube, IV fluids, Catheterisation, enemas/suppositories if constipated
Complications (3)
Perforation, toxic megacolon, ischaemic bowel disease
