What is Appendicitis?
Gut organisms invade the appendix wall following lumen obstruction
What causes luminal obstruction? (3)
Lymphoid hyperplasia, faecolith or filarial worms
What happens to the appendix? (4)
Oedema, ischaemia, necrosis and perforation
But why does it happen?
May have inability to prevent invasion e.g immunosuppression, or they are not tolerant to pathogens (Hygiene Hypothesis)
Is it common?
The most common acute surgical procedure
Who does it affect?
People aged between 10 and 20 years old
Symptoms (3)
Fever, periumbilical pain that moves to the right iliac fossa, constipation
Signs (5)
Rebounding and percussion tenderness, Rousing’s Sign (Pain in RIF>LIF), Tachycardia, flushing, Shallow breathing
What bloods would you do?
FBC (shows neutrophil leucocytosis), CRP
Other Investigations? (2)
Ultrasound, CT
Differentials (3)
Ectopic pregnancy, UTI, Anything causing peritonitis
First line treatment
Prompt appendicectomy
Other Treatment (3)
Antibiotics-usually Metronidazole and Cefuroxime, Analgesia, fluids
Complications (3)
Perforation, Appendix Mass (attached to omentum), Abscess
