Peptic Ulcer, Mallory Weiss Tear, Oesophageal Varices, Gastritis/Oesophagitis, Drugs (NSAIDs), Malignancy

A
Mostly elderly and fairly common

Dyspepsia, Ulcers, Liver disease, Drugs, Alcohol

Symptoms (5)
A
Haematemesis (fresh & coffee ground), Malaena, Dizziness, Abdominal Pain, Dysphagia

Signs (3)
A
Hypotension, Tachycardia, Reduced urine output (state of shock)

What is the first line action?
A
Rescucitation

Investigations:
A
Urgent endoscopy, monitor obs and urine output

A
Malignancy

A
IV saline, consider blood transfusion, Endoscopic haemostasis, IV PPI
