What is UC?
Inflammation of the left side of colon and rectum
What layer of the bowel does it affect?
Superficial Mucosa
Continuous or patchy?
Continuous
What type of inflammation?
Ulceration with inflammatory polyps and shows inflammatory cell infiltrate in the Lamina Propria
What histopathological features are there?
Crypt abscesses and goblet cells
Is it common?
Not really, affects 8 per 100,000
Risk Factors: (2)
Mutations to genes that control mucosal barrier e.g DRB 0103 allele
Chronic stress and depression
Any protective factors?
Smoking and Appendicectomy
Symptoms:
Chronic diarrhoea, mucus/blood in stool, cramping abdominal pain, urgency/tenesmus, weight loss, malaise, anorexia
Signs:
Tachycardia, tender abdomen, clubbing, Erythema Nodosum
Main Differentials (3)
Crohn’s, Gastroenteritis, IBS
What blood tests would you do?
FBC, ESR, CRP, U&E, LFT, Blood Culture
Other investigations:
Stool sample for faecal elastase, Abdominal x-ray, colonoscopy with biopsy
First line treatment
Mesalazine
Other treatments (4)
Prenisolone, Hydrocortisone, Ciclosporin/Infliximab therapy, resection of affected bowel
