What are the three things that allow for infection?
- Mucosal Adherence
- Mucosal Invasion
- Toxin Production
What bacteria produce toxins? (3)
Staph Aureus, Bacillus Cereus, E.coli
What causes bloody diarrhoea?
Invasins disrupt host cell cytoskeletons
What organisms cause bloody diarrhoea?
Shigella and Campylobacter
Is it common?
the most common type of acute GI infection, 2 million die worldwide from diarrhoea each year
What causes gastroenteritis in children?
Often caused by viral infections such as Norovirus or Rotavirus (quick transmission and lasts for 48 hours)
Risk Factors (3)
Contaminated food and water, water sports, immunosuppressed
Symptoms (7)
Watery/bloody diarrhoea, abdominal pain, vomiting, bloating, dehydration, weight loss, anorexia
Signs (2)
Pyrexia, Abdominal Distension
Differentials (4)
Bacterial or Viral cause, Parasitic infection, IBD, IBS
What bloods would you do?
FBC, U&E, LFT, ESR, CRP, Blood cultures
Other investigations:
Stool microscopy & culture
What is the First line treatment?
Oral rehydration solutions
Other treatments: (4)
Anti-emetics, Loperamide (not to be given in dysentry)
Antibiotics if systemically unwell, elderly or immunosuppressed, IV fluids
Complications: (4)
Haemolytic Uraemic Syndrome (esp E.Coli), Toxic megacolon, Perforation, Hypovolaemia
