SLUT FAD (COPT BSL)

A

SHBG (decreased, others increased)
LH:FSH
USS Ovaries (12 or more 2-9mm follicles per ovary, and/or ovarian volume > 10mL)
Testosterone, morning (free and total) - if very high, suspect virilising ovarian/adrenal tumour
Free androgen index
Androstenedione (marked increasing in CAH, ovarian/adrenal tumours)
DHEA-S (dehydroepiandrosterone sulfate) - if very high, suspect adrenal tumour

COPT = exclude ddx
Cortisol, 24h urinary free (Cushing’s - hyperandrogenism, amenorrhoea, metabolic syndrome)
17-OH (hydroxy) progesterone (late-onset CAH - amenorrhoea, hyperandrogenism)
Prolactin, Pregnancy test (hyperprolactinaemia)
TSH (hypothyroidism - amenorrhoea, raised BMI)

BSL = associations
BSL, BHCG, BP
Sleep study (OSA)
Lipids (+ waist circumference), LFTs (metabolic syndrome, NASH)

*must be off the OCP for 2/12, best done on day 2 - 5 period
