How to asses a pt level of alertness
A- the pt is awake and alert
V- pt isn’t fully awake will only respond to verbal commands or verbal stimuli
P- pt is difficult to rouse and will only response to pain
U- pt is completely unconscious
What is the normal resp rate for a pt
12-20 breaths
What are the dependable factors to take into account when doing a resp rate
Depth
Rhythm
Symmetry of chest
