Three causes of nephritic syndrome
Post strep glomerulonephritis
IgA nephropathy
Rapidly progressive GN
What’s seen on histology of post strep GN?
Immune complex deposition with IgG and C3
What’s seen on histology with IgA nephropathy and when does it occur?
Post URTI, asap after
IgA and C3 deposition
Difference between IgA nephropathy and post strep GN
IgA nephropathy - more frank haematuria than the other symptoms of nephritic syndrome, also immediately after
Post strep GN - all the symptoms - proteinuria, haematuria, HTN, around 6 weeks after
Three types of rapidly progressive GN
Type 1 - Good pastures aka anti-GBM
T2 - immune complex mediated from SLE, alport’s syndrome, IgA gn, post strep GN
T3 - ANCA associated
What do you see on fluorescence microscopy with all three?
T1 - linear deposition of IgG in GBM
T2 - Granular deposition of IGG
T3 - scanty immune deposition
What other organs are involved with each type of rapidly progressive GN?
T1 - Lung
T2 - none except SLE symptoms
T3 - Vasculitis
What does light microscopy show for all three types of rapidly progressive GN?
Crescents from proliferation of macrophages in Bowman’s capsule
Goodpastures HLA, what hypersensitivity reaction is it and against what?
DR15/D2
Type 2 HR
against collagen type IV
Malignant HTN causes what on renal histology
Fibrinoid necrosis
