PACES Flashcards
Where do you do pulses on a child?
Brachial and femoral pulses
What constitutes B and C
C - pulses, pre and post ductal blood pressure, HR, capillary blood gas, two large bore cannula
B - RR, oxygen, breath sounds
top oxygen you can give
100 percent non rebreathe
weak femoral pulses
aortic coarctation
What does D mean
D - pupils, glucose, temp and exposure
if you can’t get IV access in a child, what do you do?
Interosseus access
What signs do you get in coarctation of the aorta
Hepatomegaly from heart failure
Cerebral aneurysms due to high blood pressure to brain
What other pathway do you need to start as soon as a child is in shock?
Sepsis - start sepsis 6 pathway
Who do you have to call IMMEDIATELY
Anaesthetics
PICU
preductal - before ductus arteriosis
What drug would you start
Give prostaglandin infusion to open up PDA so
Causes of neonatal shock
Sepsis
Cardiac
Metabolic
NAI
What is indomethacin
Inhibits prostaglandin synthesis so helps with CLOSING of the PDA
Who gets aortic coarctation
Turner 45 XO
Webbed neck
Bicuspid aortic valve
Aortic coarctation
What do you need to give?
IV bolus (not maintenance (since it’s a heart problem)
IV Abx
Prostaglandins
