Outcome Measure (CVR Specific) Flashcards
What are some outcome measures in respiratory assessment?
- Respiratory rate
- Work of breathing/breathing pattern
- SpO2%
- ABGs
- Oxygen requirements
- Volume of sputum expectorated
- Auscultation/palpation/thoracic expansion
- CXR
What can lung function testing be used for?
- Investigate disease and function
- Measure disease progression
- Measure response to treatment
What is the Borg Scale?
a scale rating perceived exertion (RPE)
What is the Borg Scale often used as?
An outcome measure in pulmonary rehab, to monitor progress during an exercise regime + to help guide patient to exercise at a suitable level of intensity.
What did the original Borg scale measure on a scale of?
6-20
What does the modified Borg scale measure on a scale of?
1-10
What MRC stand for?
The Medical Research Council dyspnea scale
What does the MRC measure?
Levels of breathlessness during activity
What can MRC be used to identify?
The impact of breathlessness on functional level, and to compare pre/post treatment course (e.g. pulmonary rehab).
What is the 6min walk test (6MWT) useful to measure in CVR?
Measure of exercise tolerance that’s useful in chronic respiratory disease
What does the 6min walk test (6MWT) measure?
The distance walked on a hard flat surface in 6 mins, back and forth between 2 points
What does the Incremental Shuffle Walk Test (ISWT) involve?
- The patient walks around cones that are 9 metres apart in time to a set of auditory beeps played on a CD.
- The walking speed gets increasingly faster at each level.
- The test continues either when the patient feels they need to stopor if they can no longer keep up with the beeps.
- The number of shuttles (e.g. each timethe patient walks a length to a cone) is measured.
What does the endurance shuttle walk test (ESWT) involves?
- Walking between two cones.
- Theintensity is related to a percentage of each patient’s maximum field exerciseperformance assessed by the ISWT.
- Patient are to walk at the set pace for as long aspossible (max. 20minutes).
- Walk distance and walk timearemeasured.
Who would use a modified shuttle walk test (MSWT)?
Younger, fitter patients
What does the modified shuttle walk test (MSWT) involve?
- Essentially a bleep test between two cones (ISWT).
- With 3 further levels than the standardISWT.
What does CPET stand for?
Cardiopulmonary Exercise Testing
What is the CPET used for?
- Predict outcomes
- QoL and mortality risk in cardiovascular disease (especially CAD, HF and chronic CVR conditions)
What is the Chronic Respiratory Questionnaire (CRQ)?
- A self reported questionnaire that looks at the patient’s health status and health related quality of life (HRQoL).
- Made of 20 questions, split across four dimensions relating to dyspnoea, emotional function, fatigue, and mastery.
- Scores are recorded from 1-7.
- Patients identify everyday activities that make them breathless and then select, rank and score the 5 most important activities to them on the 1-7 scales.
How will a lower score in each dimension of the Chronic Respiratory Questionnaire (CRQ) impact the patient?
Lower score = greater impact on their function
