acute otitis media (AOM)
-rapid onset of fever and ear pain (otalgia)
-occurs mainly within the 1st 24 months of life
otitis media with effusion (OME)
-collection of fluid in middle ear space w/o s/s of infection
-chronic = lasting longer than 3 months
why are young children most at risk for OM?
the Eustachian tube does not curve up so fluid will sit in there and cause infection
nursing assessment of OM
-fever
-complaints of otalgia
-fussiness or irritability
-crying inconsolably, particularly when lying down
-tugging at ear
-rolling head from side to side
-poor feeding or loss of appetite
-lethargy
-difficulty sleeping
-fluid draining from ear
risk factors
-young age
-first episode of AOM before 3 months of age
-absence of infant breast feeding
-day care attendance
-passive smoking
-previous hx of AOM or OME
-antecedent or concurrent URI
-other risk factors
what kind of tympanic membrane does AOM present with?
red & bulging
complications of AOM
-perforation of tympanic membrane
-tympanosclerosis
-mastoiditis
-bacterial meningitis
-hearing loss
-expressive speech delay
