What is obstructive sleep apnoea
Tongue, tonsils, soft palate rarely can collapse -> complete AW obstruction when tone reduced in sleep -> transient O2 dip + subconscious arousal hundreds of times a night -> unrefreshing sleep, daytime sleepiness + snoring
What is a hypopnea
Partial obstruction
>10 seconds >30% redution in flow + desturation >4%
Reasons for waking
chemoreceptors sense drop of oxygen → kick of adrenaline to stimulate rep muscles + wake up a little to open AW
Alos → tachycardia, swings in BP
Symptoms of OSA
- Excessive daytime sleepiness
- Snores or stops breathing overnigth
- Dropping off during the day
- Increased BMI and larger neck sizes
- Craniofacial abnormalities or narrow nasopharynx
What score is used on OSA
Epworth sleepiness score and Stop bang
Investigations ofr OSA
Polymnography
EEG/EMG
Sats oxygen, HR, airflow
Treatment for OSA
Weigth loss
Wean opiates/benzos
CPAP
Mandibular advancement devices
