Heart block Flashcards
What is first degree heart block
> 200ms PR interval
What do in first degree HB
Nothing
Take care when using rate slowing drugs
Mobitz type I
Prolongingly prolonged PR interval with dropped beat and repeated cycle
Mobitz type 2
2:1 or 3:1 relationship p waves to QRS complex
How to differentiate Mobitz type I vs II with symptoms
I - young people at night, goes with exercise
II- progressive condition ass with older age, doesnt resolve with exercise
When does there become a risk of asystole with heart block
Mobitz type II - fit a pacemaker
Complete heartblock
No relationship between P waves and QRS
Regular p waves
Medical emergnecy
How does AF with complete heart block present
Baseleine - no clear p wave
AF with slow regular ventricular rhythm - complete HB
medical emergency
Incomplete vs complete bundle branch block
Complete QRS >120ms
Incomplete BBB <120ms but QRS changes
What is seen on ECG in RBBB
MaRroW V1-V6
What causes RBBB
Idiopathic
MI
cardiomyopathies
Lung/pulm circulation eg COPD/PE/RHF
Why are waves M or W in ECG BBB
separate polarisation of R vs L ventricle depending on which bundle branch is delayed
Progression of LBBB vs RBBB
LBBB v likely to progress to complete whereas RBBB isnt
ECG of LBBB
WiLliaM -V1-V6
