Coordination scoring:
What is the range of scoring?
Describe.
0-4 0= unable 1= severe impairment 2= moderate impairment 3= minimal impairment 4= NORMAL
DTR Scoring
What is the range?
Describe.
0 to 4+
0 = absent
1+ = tone change, no visible movement of extremities
2+ = NORMAL - visible movement
3+ = exaggerated, full movement of extremity
4+ = obligatory and sustained movement, >30 seconds
Modified Ashworth Scale
Describe.
0 = no increase in muscle tone 1= slight increase in muscle tone, minimal resistance @ end ROM 1+ = slight increase in muscle tone, minimal resistance thru < 1/2 ROM 2= marked increase in muscle tone through most ROM, affected part easily moved 3= considerable increase in muscle tone, passive movement is HARD 4= rigid in flexion or extension
Functional Balance grades
What are the grades?
Normal, good, fair, poor, 0 absent
Functional balance grades:
Describe normal
Pt is able to maintain steady balance w/o hand hold support - static
Pt accepts MAX challenge and can shift weight easily at full range in all directions - dynamic
Functional balance grades:
Good
Pt is able to maintain balance w/o hand hold support, LIMITED POSTURAL SWAY - static
Pt accepts MODERATE challenge; able to maintain balance while (picking object off floor) - dynamic
Functional balance grades:
Fair
Pt is able to maintain balance WITH hand hold support; may require occasional MIN ASSIST - static
Pt accepts MINIMAL challenge; able to maintain balance while (turning head/trunk) - dynamic
Functional balance grades:
Poor
Pt requires hand hold support and MOD to MAX assistance to maintain position - static
Pt UNABLE to accept challenge or move without LOB - dynamic
Functional balance grades:
Absent 0
Pt unable to maintain balance
