Indications for Nitroglycerin
Acute angina pectoris Ischemic chest pain Hypertension CHF Pulmonary Edema
Action of Nitroglycerin
Smooth muscle relaxant acting on vasoculature, bronchial, uterine, intestinal smooth muscle.
Dilation of arterioles and veins in the periphery.
Reduces preload and afterload, decreasing workload of the heart and thereby myocardial oxygen demand
Contraindications of Nitroglycerin
Hypotension Hypovolemia Intracranial Hemorrhage Pericardial tamponade Severe bradycardia or tachycardia RV infarction Previous administration in last 24 hours: tadalafil (Cialis) (48 hours), vardenafil (Levitra), sildenafil (Viagra)
Adverse Effects of Nitroglycerin
Headache Dizziness Weakness Reflex tachycardia Syncope Hypotension Nausea Vomiting Dry mouth Muscle twitching Diaphoresis
Precautions for Nitroglycerin
Not to be given if patient had ED drugs within past 48 hours
Dose of Nitroglycerin
Adult: 0.4 mg SL May repeat in 5 minutes to a max of 3 doses.
IV infusion 10 mcg/Mom increase by 10 mcg/min every 3-5 minutes until desired effect with a max dose of 200 mcg/min
Pedi: Not recommended
