Mandatory Inclusions

A

When conducting a Sight test, which is defined in Law, you must perform an internal and external examination and carry out such additional examinations as appear to be necessary to detect signs of injury, disease or abnormality in the eye or elsewhere.

You should use your professional judgment and the minimum legal requirements to decide the format and content of the tests.

You should allow sufficient time to perform the examination.

You must record all clinical findings. You must do this legibly and at the time of the examination, or as soon as possible afterwards.

NHS Eligibility Criteria -

You are entitled to a FREE NHS Sight Test if you:

A

1) Are under 16 = Child
2) Are 16,17 or 18 and in Full-time Education
3) Are 60 or over = OAP
4) Are registered as partially sighted or blind.
5) Have been diagnosed with Diabetes or Glaucoma.
6) Are 40 or Over, and your mother, father, sibling or child has been diagnosed with Glaucoma.
7) Have been advised by an eye doctor (Ophthalmologist) that you’re at risk of Glaucoma.
8) Are a prisoner on leave from Prison?
9) Are eligible for an NHS complex lens voucher – you Optician can advise you about your entitlement.

You can also qualify for a FREE NHS Sight Test if you:

A

10) Receive Income Support
11) Receive Income-based Jobseeker’s Allowance.
12) Receive Income-based Employment and Support Allowance.
13) Receive Pension Credit Guarantee Credit.
14) Receive Tax Credits and meet the criteria.
15) Get Universal Credit and meet the criteria.
16) Have a low income and are named on a valid NHS HC2 certificate for full help with health costs.
17) You may be entitled to a reduced cost sight test if you have a low income and are named on a valid NHS HC3 certificate for partial help with health costs.

Tax Credit Criteria

A

You are entitled to full help with Health costs if your annual family income used to calculate your Tax Credits is £15,276 or less and you receive either:

  • Child Tax Credit
  • Working Tax Credit and Child Tax Credit paid together
  • Working Tax Credit including a disability or severe disability element

Universal Criteria

A

You qualify if, on the date you claim help with health costs:

a) You receive Universal Credit and either had no earnings or had net earnings of £435 or less in your last Universal Credit assessment period.
b) You receive Universal Credit, which includes an element for a child, or you (or your partner) had limited capability for work (LCW) or limited capability for work and work-related activity (LCWRA), and you either had no earnings or net earnings of £935 or less in your last Universal Credit assessment period.

NHS Complex Lens Voucher

A

To be eligible, your lenses need to be either +/-10D or more or prism controlled Bifocal lenses.

Complex lens vouchers are a contribution towards the cost of these lenses – currently £14.60 for single vision lenses and £37.40 for bifocal lenses.
