Olfactory -what nerve number and test
one and “has your smell changed”
Optic what nerve number and test

each eye
-visual acuity (snellen test)
-pupillary light reflex (direct, consensual, accommodating)
-assess visual fields (check the 3 quadrants of vision with Bunnies, one double bunny and then cover one eye at a time for peripheral vision)

H sight test-checking for nystagmus, diplopia or blurred vision)
trigeminal what nerve number and test

test facial sensation (forehead, cheeks, and mandible)
clench jaw and assess muscle bulk and tone on temporal region and masseter region

seven
- ask “any changes too taste”
- raise eyebrows (can’t do this if bell’s palsy
- puff out cheeks, and attempt to deflate them by poking
- assess for smile ability “big smile expose your teeth”
vestibulocochlear what nerve number and test
eight
stand behind patient, distract one ear (rubbing fingers), then lean approx 1m away and whisper number into other ear, have patient repeat number, test both ears
glossopharyngeal

nine, ten
has anything changed in your swallowing or voice
assess uvula, have patient open mouth, assess uvula is midline and symmetrical, have patient stick out tongue and say ah, ensuring uvula rises symmetrical and stays midline (if deviation, will move in direction opposite lesion)
accessory what nerve number and test
eleven
inspect trapezius muscles, have patient shrug once, then shrug again and stop you from putting shoulders down (assess strength)
assess sternocleidomastoid muscle, have patient turn head then push back on hand (test strength)
hypoglossal what nerve number and test
twelve
open mouth and stick out tongue, assess symmetry and for one sided weakness/ ability to do so.
have patient push tongue into check and push back on tongue, assessing strength
neuro global impression and post cranial nerve test
look for
- scars
- cachexia
- fasciculations
- tremors
- drooping
- speech abnormalities
- rashes
- pain (neck)
post exam, perform upper and lower neuro test
