What are drugs for withdrawals?
disulfiram (Antabuse)
naltrexone
acamprosate
methadone, buprenorphine
Buspirone
Non-benzo
Schedule, 2-6 weeks for effect
Tx: anxiety, panic, OCD
S/S: CNS effect
Amitriptyline (TRICYCLIC ANTIDEPRESSANT)
S/S: anticholinergic, orthostatic hypotension, sedation, sweating, serotonin syndrome (fever, disorientation, seizure)
phenelzine, selegiline (MAOI)
Don’t take with tyramine food – leads to hypertensive crisis
Don’t take with other antidepressants within 2 weeks of starting/stopping
SSRI and SNRI
Fluoxetine (Prozac)
Therapeutic effects 4-6 weeks
Monitor for suicidal ideation
S/S: weight gain, decreased libido, serotonin syndrome
Avoid NSAIDs
What is serotonin syndrome?
hyperthermia, seizures, dysrhythmias, renal failure, DIC
Bupropion (wellbutrin)
Tx: SAD, smoking cessation
Similar to SSRI
S/S: weight loss, seizures
Lithium carbonate (mood-stabilizer)
Non-addictive
Drink 2-3L
Therapeutic range 0.6-1.2
Early toxicity s/s: coarse tremors (fine tremors expected), confusion, GI distress
Moderate toxicity: ataxia, hypotension, seizures
Late toxicity: oliguria, coma, death
“<1.5 to stay alive”
Monitor sodium lvls
Toxicity treatment: gastric lavage, hemodialysis <– only when super bad
Chlorpromazine, haloperidol
Tx: schizophrenia
Short-term treatment
S/S: Extrapyramidal symptoms, Neuroleptic syndrome, anticholinergic
What is neuroleptic syndrome?
hyperthermia, stupor, VS elevation
What is extrapyramidal symptoms
acute dystonia, tardive dyskinesia, pseudo-parkinsonism, akathisia
Risperidone, quetiapine, aripriprazole
S/S: metabolic syndrome, anticholinergic
Tx: schizophrenia
What is metabolic syndrome
hyperlipidemia, insulin resistance, diabetes mellitus, obesity
betaxolol, timolol
Tx: treatment of glaucoma
S/S: respiratory depression, bradycardia, hypotension
Administer to lower conjunctival sack and close for about 30 seconds
