clinical interview: what brought them into the clinic?
chief complaint
clinical interview: how did the symptoms start, evolve, anything associated with it?
history of presenting illness
clinical interview: inquire about the patient prior to the illness
medical history
family history
social history
clinical interview: examine all major organs
general examination
neurological examination is organized into 5 parts
- mental status and higher functions
- cranial nerves
- motor system (gain, bulk, tone, power, coordination, adventitious movements)
- sensory system
- reflexes
3 parts to the cognitive evaluation
- formulating the clinical vignette
- localize
- possible etiologies/diagnosis
formulating the clinical vignette
integrate all the information from the clinical interview
a cohesive abstraction of what’s going on
localize
the region - tell where the problem is
advantages: make diagnostic testing more specific, can give clues about etiology
potential etiologies/diagnosis
use vignette + location to get differential diagnosis (list of possibilities)
set of hypothesis that can then be tested
symptoms
subjective - patient reports this
sign
objective - you watch and see it
acute
happens because blood or electricity messes up in the brain
subacute
occur by inflammatory, infection or some cancers
chronic
cancers, alzheimer’s
