Muscle = Shoulder abduction - Deltoid (Axiallary.n)
Forearm Flexion - Biceps (Musculocutaneous)
Reflex = Biceps
Sensations = Lateral arm
Nerve Root C6
Disc level = C5
Muscle = Wrist extension - Ext carpi radialis longus & brevis & extensor carpi ulnaris (radial nerve)
Reflex = Brachioradialis
Sensation = Anterior lateral forearm, palm, thumb, and 2nd digit
Nerve Root C7
Disc Level = C6
Muscle = Elbow extension - triceps (radial.n)
Wrist flexion - Flexor carpi radialis (median.n), flexor carpi ulnaris (ulnar.n)
Finger extension - extensor digitorum communis, extensor indicis profundus, extensor digiti minimi (radial.n)
Reflex = Triceps Sensation = 3rd, middle of palm
Nerve Root C8
Disc Level = C7
Muscle = Finger Flexion - flexor digitorum superficialis, flexor digitorum profundus, lumbricals (median & ulnar.n)
Reflex = none Sensation = 4th, 5th digits, antero-medial hand and forearm.
Nerve Root T1
Disc level = C8
Muslce = Finger Abduction - dorsal interossei (ulnar.n)
Finger Adduction - palmar interossei (ulnar.n)
Reflex = none
Sensation = Antero-medial arm (distal aspect of arm to proximal aspect of forearm)
Nerve root L4
Disc level = L3
Muscle = foot Dorsiflexion & inversion: tibialis anterior or [deep fibular nerve]
Reflex = patellar tendon
Nerve root L5
Disc level = 04
Muscle = foot Dorsiflexion
Big toe Dorsiflexion: extensor Hallicus longest [deep fibular nerve].
Toes 2,3,4 Dorsiflexion: extensor digitorum longest & brevis [deep fibular nerve].
Hip and pelvis abduction: gluteus medias & minimus [superior gluteal nerve]
Reflex = none
Nerve root S1
Disc level = L5
Muscle = foot plantar flexion: gastrocnemius and Solias [tibial nerve]
Foot plantar flexion and eversion: fibular/Peroneus is longus & brevis [superficial fibular nerve]
Hip extension: gluteus maximus [inferior gluteal nerve]
Reflex = Achilles
