what is the relationship of Hypertension and Hyperkalemia?
Think renal artery stenosis. Low potassium dilates and lowers BP.
effect of aldosterone on potassium?
Aldosterone causes more potassium to be drawn out of body into the urine.
workup for Secondary HPT due to CKD
25 OH vitamin D, Calcium, Phosphate, Alk Phos, Possible DEXA,
Treatment for Hyperphospatemia (non diaylsis ideal range 3.5-4.5)
1-Phosphate restriction: 900mg per day or less, Processed foods, Soda
2- phosphate binders
who should have Renal Ultrasound in CKD
pt <45 yrs with CKD 1 or 2, Ultrasound shoulder length >16.5 cm closely assoc with rapid progression to stage 3 within 8 yrs. or do MRI or CT measure kidney volume.
