What is involved in the introduction of a neck lump exam?
A
- Wash/gel hands
- Introduce yourself, confirm pt, explain examination and gain consent
- Expose patient (entire neck) - unbutton or remove top

What would a thin, fidgety, sweaty, flushed, restless patient from end of bed suggest?
A
hyperthyroidism

What would a fat, warmly dressed, hair loss, dry skin, deep voice patient from end of bed suggest?
A
hypothyroidism

What are the stages of inspection for a neck lump?
A
- Look from front to sides
- Define location: midline, anterior / posterior triangle
- Swallowing
- Stick out tongue

What do you ask about swallowing?
A
- Ask pt to sip of water and hold it in mouth
- Instruct to swallow as you look from front
- Instruct to swallow as looking from side

Why do you ask the patient to stick out their tongue?
A
thyroglossal cyst moves upwards

What do you do in palpitation of neck lump?
A
- stand behind patient use both hands to examine
- palpate from behind whilst patient swallows
- attempt to transluminate lump

What do you look for when palpating lump?
A
- Start in centre of lump and move out to edges
- Size/shape/symmetry
- Surface
- Consistency
- Edge
- Fluctuance
- Palpation

What do you check specific to thyroid swelling when palpating neck?
A
- Diffuse enlargement or single nodule?
- If diffuse is it smooth or multinodular
- Lower extent - can you get below the thyroid
