What are some causes of hepatomegaly?
- Hepatitis (infective and non-infective)
- Hepatocellular carcinoma
- Hepatic metastases
- Wilson’s disease
- Haemochromatosis
- Leukaemia
- Myeloma
- Glandular fever
- Primary biliary cirrhosis
- Tricuspid regurgitation
- Haemolytic anaemia
What would a palpable gallbladder suggest?
- enlargement secondary to biliary flow obstruction (e.g. pancreatic malignancy, gallstones)
- and/or infection (e.g. cholecystitis)
What is Murphy’s sign?
- Position your fingers at the right costal margin in the mid-clavicular line at the liver’s edge.
- Ask the patient to take a deep breath.
What is Murphy’s sign positive?
patient suddenly stops mid-breath due to pain, this suggests the presence of cholecystitis
What are causes of splenomegaly?
- Portal hypertension secondary to liver cirrhosis
- Haemolytic anaemia
- Congestive heart failure
- Splenic metastases
- Glandular fever
What would bilaterally enlarged kidney suggest?
- polycystic kidney disease

What is a unilaterally enlarged ballotable kidney caused by?
renal tumour
What are tinkling bowel sounds suggestive of?
bowel obstruction
What would absent bowel sounds be because of?
- ileus
- due to electrolyte abnormalities and recent abdominal surgery
- need to listen for 3 mins to decide
What would pitting oedema on lungs suggest?
hypoalbuminaemia (e.g. liver cirrhosis, protein-losing enteropathy)
What would a bruit with aortic bruits suggest?
AAA
What would a bruits near renal bruits suggest?
renal artery stenosis
