What should be mentioned in cardio systems review?
- Chest pain
- Shortness of breath (exertional, orthopnoea, PND)
- Palpitations
- Syncope
- Ankle swelling
- (think of nausea, vomiting and sweating if presenting with chest pain)
What should be mentioned in a resp systems review in a cardio history?
- (may be relevant if presenting complaint is chest pain or shortness of breath)
1. Cough (+ ?sputum)
2. Haemoptysis
3. Wheeze
4. Shortness of breath
5. Fever
6. Chest pain (?pleuritic)
What should be mentioned in gastro systems review for a cardio history?
- targeted towards GORD
1. Worse after meals or on lying down?
2. Relieved with Gaviscon
What general things must be covered as part of systems review in cardo history?
- Malaise / lethargy
- Appetite change / weight loss
- Fevers / night sweats
- Rashes / skin changes
What modifiable risk factors should you ask about in a cardio history?
- Diabetes mellitus
- Hypertension
- Hypercholesterolaemia
- Diet and exercise
- Smoking
What non-modifiable risk factors should you about in cardio history?
- Age
- Gender (male)
- Family history
- Nausea and vomiting
What social history questions should you enquire about in cardio history?
- Smoking, alcohol, recreational drugs
2. Occupation, living situation, ADLs
What could possible causes of palpitations be?
- drugs
- caffeine
- alcohol
- infection
- cardiac disease
- thyroid symptoms
-Check effect on activities of daily living
If the patient has experienced syncope what must you ask?
- Any evidence of pre-warning / preceding symptoms
- Any witnesses to the event
- What the recovery period was like after regaining consciousness
-note if the patient has an occupation / hobby where a sudden syncopal episode would be especially dangerous e.g. diving / swimming / driving
What do you need to ask about SOB?
- chronicity
- orthopnoea
- ankle swelling
- PND
