Atypical pathogens that cause pneumonia
- Legionella
- M. pneumoniae
- Chlamydia pneumoniae
- Chlamydia psittaci

Walking pneumonia, seen in young adult and its long term, wheezing in someone who doesn’t have asthma, pleural effusion
Mycoplasma pneumoniae diagnosis
Clinical suspicion best for immediate result, if available serology test 7-9 days
Mycoplasma pneumoniae treatment
Azithromycin or doxycycline or cipro
Chlamydia psittaci
Spread via birds to human, “parrot disease”, high fever, abrupt onset
Chlamydia pneumoniae
Not very common source of pneumonia, transmission from person to person, gradual onset, pharyngitis, can lead to meningoencephalitis, gulliain barre
Chlamydia pneumoniae diagnosis
Rare to establish diagnosis, nasopharyngeal swabs
DOC for chlamydia pneumoniae
Doxycycline twice a day orally
Legionnaire’s disease transmission
Person to person contact never been demonstrated
Legionnaire’s distinct clinical presentation from other pneumonais
Diarrhea, nausea, vomitting
Legionnaire’s disease diagnosis
Legionela urinary antigen test, legionella culture of sputum
Legionnaire’s disease treatment
Fluorquinolone or azithromycin
