- Suspect ankylosing spondylitis
- Check HLA-B27

Reactive (Reiter’s) arthritis
-A/w Campylobacter, Shigella, Salmonella, Chlamydia, and Ureaplasma
55 yo M has sudden, excruciating 1st MTP jt pain after a night of drinking red wine. Dx? W/u? Chronic tx?
- Dx: gout
- W/u: needle-shaped, negatively birefringent crystals seen on jt fluid aspirate
- Chronic tx: allopurinol or probenecid
Rhomboid-shaped, positively birefringent crystals on jt fluid aspirate
Pseudogout
Elderly F presents with pain and stiffness of the shoulders and hips. She cannot lift her arms above her head. Labs: anemia and increased ESR.
Polymyalgia rheumatica
Active 13 yo boy has anterior knee pain. Dx?

B. is fx’d in a fall on an outstretched hand

- Tx: if severe, consider a Pavlik harness to maintain ABDuction
11 yo obese AAM presents with sudden onset of limp. Dx? W/u?
- Dx: slipped capital femoral epiphysis (SCFE)
- Tx: AP and frog-leg lateral view
