PHARMACOLOGY OF MORPHINE
An opioid analgesic
PRESENTATION OF MORPHINE
10 mg in 1 mL glass ampoule
ACTIONS OF MORPHINE
CNS EFFECTS:
- CNS depression
- Respiratory depression
- Depression of cough reflex
- Stimulation ( changes of mood, euphoria or dysphoria, vomiting, pin-point pupils)
- Dependence (Addiction)
CARDIOVASCULAR EFFECTS:
- Vasodilatation
- Decreases conduction velocity through the AV node
PRIMARY EMERGENCY INDICATIONS OF MORPHINE

- Pain relief
- Acute LVF with SOB and Full field crackles
- Sedation to maintain intubation
- Sedation to enable intubation
- RSI
CONTRAINDICATIONS OF MORPHINE
- Known hypersensitivity
2. Late second stage labour
PRECAUTIONS OF MORPHINE
List all 8
- Acute alcoholism
- Current Asthma
- Elderly Pts
- Hypotension
- Respiratory depression
- Respiratory tract burns
- Known addiction to opioids
- Pts on monoamine oxidase inhibitors

IV
IM

SIDE EFFECTS OF MORPHINE
CNS EFFECTS: - Drowsiness - Respiratory depression - Euphoria - Nausea & vomiting - Addiction - Pin-point pupils CARDIOVASCULAR EFFECTS: - Hypotension - Bradycardia
ONSET & DURATION FOR MORPHINE
IV
Onset: 2 - 5 min
Peak: 10 min
Duration: 1 - 2 hr
IM
Onset: 10 - 30 min
Peak: 30 -60 min
Duration: 1 - 2 hr
*Note: On occasion wheals may be seen in the line of the vein, this is not an allergy only a histamine release
MORPHINE ADULT DOSE
PAIN RELIEF:
IV
Up to 5mg, Repeat 5mg @5/60 if required (Max 20mg)
IM
Pt >60kg 10mg , Repeat 5mg @ 15/60 once if required
Pt 7 2.5mg IV @5/60 (Max 20mg)
*Note only give after 2XPenthrax and Stemetil & >15min from destination hospital
