What are the most common causes of hemorrhage in early pregnancy?
- Abortion (spontaneous)
- Ectopic Pregnancy
- Gestational trophoblastic disease (GTD)
Spontaneous Abortion
Occurs in about 15-20% of diagnosed pregnancies. Unintended loss of pregnancy before the fetus is viable or capable of living outside the uterus:
- Prior to 20 weeks gestation
- Less than 500g
Therapeutic Abortion
Medically-induced termination of pregnancy
Causes of Spontaneous Abortion
- Most common cause: severe chromosomal abnormalities
- Other causes: maternal infection, endocrine disorders, anatomical defects (example: incompetent cervix)
Signs and Symptoms of Spontaneous Abortion
- Vaginal bleeding or “spotting”
- Uterine cramping, persistent backache, or pelvic pressure
- Leaking fluid
Therapeutic Management of Spontaneous Abortion
- Confirm pregnancy by ultrasound and hcg levels
- Wait and see what happens
- Monitor bleeding
- Vacuum extraction or dilation and curettage if needed
- Follow up:
- Rhogam
- Counseling
- Cerclage
